Basal lung crackles
Presence of the third heart sound
right sided heart failure features
Jugular venous distension
Hepatomegaly
Peripheral oedema
NO LUNG CRACkLES
MC symptom of PHTN
dyspnea
main therapies
Give O2, anticoagulate, dilate pulmonary artery meds
Endothelin causes vasoconstriction, we want to block this
Or upregulate nitric oxide and prostacyclin pathway
When there is hypoxemia, systemic vascular resistance goes down (systemic hypotension) and pulmonic vascular resistance goes down (PHTN)
Group 1
-O2, diuretics, anticoagulation, selective vasodilators
Group 2
-mgmt. of CHF
Group 3
-treat underlying lung condition, Give O2 and diuretics
Group 4
-from unresolved clots, anticoagulated life long, give O2 and diuretics
Cor Pulmonale
Right Sided Heart Disease, secondarily caused by abnormalities of lung parenchyma, airways, thorax, or respiratory control mechanisms.
