1-either slight pain or slight redness near site(observe)
2-pale near site, erythema, swelling(two options evident)-(resite cannula)early stage
3-pain, erythema, induration(all evident) -(resite and consider treatment) medium stage
4-pain,erythema, induration, palpable venous cord(all evident)- (resite, consider treatment) advanced stage or thrombophlebitis
5-pain, erythema, induration, palpable venous cord, pyrexia(all evident)- advanced stage of thrombophlebitis( initiate treatment, resite cannula)
