A

-Score of 0- monitor minimum 12h, continue routine NEWS monitoring

-Score of (1-4)- minimum 4-6h, inform RN and decide if frequency of monitoring will be increased, Escalation with critical care

-Score of (5 or more)- hourly, RN urgently inform medical team, Urgent assessment by a clinician, critical care with monitoring facilities, admisdion to ICU

Score of (7 or more)- continuous monitoring VS, RN immediately inform medical team(specialist registrar level), emergency assessment by medical team with critical care competencies(with advance airway skills), transfer to an level 2 or 3 care facility
