Phenothiazines Therapeutic Actions
• Depresses various areas of the central nervous system (CNS)
Phenothiazines Indications
- Treatment of nausea and vomiting
Phenothiazines Contraindications and Cautions
Contraindications:
• CNS depression: coma, brain damage or injury
• Severe hypotension or hypertension
• Severe liver dysfunction
Caution: • Renal dysfunction • Moderate liver impairment • Active peptic ulcer • Pregnancy and lactation
Phenothiazines DDI
DDI:
• Other CNS depressants, including alcohol
Phenothiazines Adverse Effects
- CNS effects: Drowsiness, dizziness, weakness, tremor, headache, photophobia
- CV effects: Hypotension, hypertension, cardiac arrhythmias
- GU effects: Menstrual disorders, galactorrhea, and gynecomastia
Phenothiazines:
Assessment
Hx: Allergy, pregnancy and lactation, impaired renal or hepatic function, active peptic ulcer disease, assess complaints of nausea
Physical: vital signs, orientation, affect, reflexes; coma, semiconscious state, CV status, assess abdomen and evaluate emesis; note color, amount and frequency of vomiting episodes (baseline)
Labs: monitor results
Phenothiazines:
Diagnosis
- Acute pain
- Risk for injury
- Decreased cardiac output
- Deficient knowledge regarding drug therapy
Phenothiazines:
Implementation
- Assure that the route of administration is appropriate
- Assess the patient carefully for any potential drug–drug interactions
- Provide other measures to help the patient cope with the discomfort of nausea and vomiting and drug effects
- Provide thorough patient teaching
