Pharmacology: Tuberculosis Flashcards
What is the duration of treatment for TB
Intensive phase: 2 months RIPE
Continuation phase: 4 months RI
What is the duration of extra pulmonary TB treatment
Intensive phase for 2 months
Continuation phase for 7 months
Why is the treatment for TB so long
To kill all the bacteria (semi dormant)
What is 2 reasons for multi drug use in TB
Prevent drug resistance
Work in different environments & kill in different ways
What is multi drug resistance (MDR)
Resistance to rifampicin & isoniazid
What is extra drug resistance (XDR)
Rifampicin, isoniazid & 2nd line
What is the 3 mechanisms of drug resistance
- Decrease permeability of drug or increase drug efflux
- Enzymes that degrade compounds
- DNA mutation
What bacteria is at the cavities
Rapid dividing
What bacteria is at the cassation
Slow dividing/persistent bacteria due to low pH & oxygen
What is 4 drugs used for TB treatment
Rifampicin
Isoniazid
Pyrazinamide
Ethambutol
What is 2 characteristics of rifampicin
Sterilizing agent against persistent bacteria
Potent inducer that decrease other drugs concentration
What is the mechanism of action of rifampicin
Inhibit RNA polymerase by binding to nuclear pregnane X receptor
What is the 3 side effects of rifampicin
DILI
Hypersensitivity
Body fluid discoloration
What is characteristic of isoniazid
Acetylar can be slow that cause toxicity or fast that cause sub therapeutic concentration
What is the mechanism of action of isoniazid
Inhibit mycolic acid synthesis
