Common student errors
- Not reconciling meds-check order, MAR
- Not monitoring vital signs (remember video ex or patient that was hemorrhaging)
- Giving oral meds as injection
- Wrong patient
- Wrong rate
- Mixed med incorrectly
- Gave wrong drug
- Wrong time

- Near miss (error that did not make it to the patient)
- Error that causes harm
- Error that causes no harm
- Error that causes death
