MoA azole anti-fungals
they compete with CP450 which is a key enzyme in fungal ergosterol synthesis. Accumulation of methylated sterols in the cell results in weakening of the cell membrane and increased cell permeability.
MoA griseofulvin
Inhibition of fungal mitotic spindle formation
Terbinafine MoA
Also inhibits ergesterol synsthesis via blcoking of squalene monoxygenase
MoA Amphotericin B
Binds membrane sterols, altering membrane permeability.
Should anti-fungal agents be given with food? Why?
They need to be given with a meal to stimulate gastric acid release so that they can be better absorbed.
Which anti-fungals have a greater effect on steroid synthesis?
Imidazoles:
- Ketoconazole
- clotrimazole
- Enilconazole
- Miconazole
What is the main concern/side effect with using amphotericin B?
It has a high risk of causing AKI
