What drugs can be used to inhibit CRH secretion at the level of the hypothalamus? What effect will this have?
Bromocriptine and Peroglide (DA Agonists). - Decrease in ACTH and cortisol levels.
What drugs can be used to stimulate CRH secretion at the level of the hypothalamus? What effect will this have?
Cyproheptadine. - Increase ACTH and cortisol levels.
What drug can be used to inhibit ACTH secretion at the pituitary level? How does it work?
Dexamethasone (glucocorticoid analogue) - inhibits ACTH by negative feedback.
What drugs can be used to stimulate adrenal synthesis of cortisol?
ACTH analogues e.g. Synacthen.
What drugs can be used to inhibit adrenal synthesis of cortisol?
Aminoglutethimide, Metyrapone.
What drug can be used to destroy the adrenal cortex? Where does it affect?
Mitotane causes destruction of the ZF and ZF.
What drug can be used to mimic Cortisol?
Dexamethasone.
What drug can be used to mimic Aldosterone?
Fluorocortisol.
What are the side effects of drugs mimicking Cortisol?
Adrenal gland atrophy, Hypoadrenocorticism if stopped suddenly, Iatrogenic Cushing’s, PU, PD due to inhibition of ADH action.
What drug can be used to increase TSH release from the pituitary?
Protirelin (synthetic TRH) - used to test thyroid dysfunction (TRH stimulation test)
What drugs can be used to decrease TSH release from the pituitary?
Thyroxine or Triiodothyronine. - Used to test thyroid gland function (T3 suppression test)
What drug could be used to increase Thyroid hormone production in thyroid gland? Why is it not used?
TSH could be used, but it is not liscensed. It can be used to diagnose thyroid disorders.
What drugs can be used to decrease Thyroid hormone synthesis from the thyroid gland?
Radioisotopes of Iodide. - used to treat Hyperthyroidism. Also anti-thyroid drugs.
Give some examples of anti-thyroid drugs. What do they do?
Perchlorate & Thiocyanate compete with I for uptake by pump. Thioureylenes block TPO. Inorganic iodide can be used to manage hyperthyroidism.
What drug can be used to mimic T3 in Hypothyroid therapy?
Sodium Liothyronine (T3 ‘Tetroxine’)
