What is hyperuricemia?
uric acid conc in plasma over 7mg/dL
What are the causes of hyperuricemia?
- Metabolic (10%)primary- associated with enzyme abnormalitiessecondary- increased purine biosynthesis2. Renal (90%)
What are the types of renal causes of hyperuricemia?
a) primary- renal failureb) secondary- long term diuretic use or toxemia or pregnancy
What are the therapeutic aims of gout treatment?
- Terminate an acute attack first2. Prevent recurrence of acute gouty arthritis3. Reverse or prevent complications of deposited ureate crystals4. Prevent other factors associated with the disease5. Prevent kidney stones
What is colchicine?
drug used as an alternative for NSAIDs thatdepolimerizes microtubules for treatment of acute gouty arthritis (prophylatic agent against these attacks)
Other therapeutic uses of cholchicine?
-Familial Mediterranean Fever-a fib-pericarditis
Side effects of acute use of colchicine?
GI disturbances (dose limiting AE-common at high doses)
Chronic AE of colchicine use? Contraindications?
blood dyscrasia due to halting cells during mitosis and resultant neutropenia-peripheral neuropathyContra: macrolides (can lead to cholchicine toxicity-GI pain, fever, myalgia, organ failure)
What is indomethacine (Indocin)?
a potentNSAID Cox inhibitor with analgesic andantipyretic and primarily anti-inflammatory properties (should not be used for minor aches and pains or fever)
What is Indomethacine used for in gout?
acute attack treatmentgive 50mg 3x daily with antacid
AEs of Indomethacine?
N/V, GI ulcers (might want to give a PPI)-lithium retention (like other NSAIDs)-Severe CNS frontal headache sometimes with vertigo or blurred vision-effects on fetal heartSide effects are pretty severe so Indomethacine is used primarily only on a short term basis
What drugs can Indomethacine antagonize?
Furosemide and HCTZ-warfarin (can see raised INR)
What does allopurinol do?
competitive inhibitor of xanthine oxidase
What are the effects of allopurinol?
It increases plasma levels of uric acid precursors such as xanthine and hypoxanthine-facilitates dissolution of uric acid crystals in the joints and kidneys
What kinds of gout is allopurinol good for?
-primary hyperuricemia of gout due to enzyme abnormalities and in children with familial juvenile hyperuricemia nephropathy-secondary hyperuricemia due to heme disorders (such as MM) or chemo
