Antiarrhythmic classes known to prolong QT interval.
Class IA (quinidine, procainamide, disopyramide) Class III (amiodarone, sotalol)
Main side effect of Quinidine.
Diarrhea
Main side effect of Procainamide.
Lupus-like symptoms
Why doesn’t lidocaine treat atrial fib or tachycardia?
Only works on ventricles.
3 beta blockers known to improve survival in HF patients.
- Metoprolol Succinate (extended release)
- Carvedilol
- Bisoprolol
Indication for Dofetilide.
Atrial fib and flutter
Only drug proven to decrease mortality in ventricular tachycardias.
Sotalol
-however overuse can prolong QT and lead to TdP
To prevent sudden cardiac death in patients, what is the mode of treatment for patients that have:
- MI
- V. tach or V. fib
- Patients with V tach undergoing CABG
- Beta blockers
- Defibrillator + amiodarone or sotalol (if patients decline a defibrillator just give the meds)
- Already doing a heart surgery so just ablate the hyperactive automaticity sites
Best treatment for TdP.
Magnesium
-can also do a pacemaker if they don’t want medication
Drug of choice for paroxysmal supraventricular tachycardia (PSVT).
Adenosine
Main ACE-I that causes rash.
Captopril: caused by sulfhydryl functional group
Best diuretic to give a HF failure patient with renal insufficiency and sulfa allergies.
