Leuprolide
M: GnRH analog – agonist (pulsatile) or antagonist (continuous)
C: Pulsatile (infertility) or continuous (prostate cancer, uterine fibroids, precocious puberty)
Estrogens
C: Hypogonadism or ovarian failure, menstrual abnoramlities, hormone replacement
T: Increased risk of endometrial cancer – NO FOR ER+ BREAST CANCER OR HISTORY OF DVT
Clomiphene
M: Antagonist at estrogen receptors in hypothalamus – increase release of LH and FSH
C: Infertility due to anovulation
Tamoxifen
M: Antagonist at breast, agonist at bone and uterus
C: ER/PR+ breast cancer
T: Increased risk of endometrial cancer
Raloxifene
M: Antagonist at breast and uterus, agonist at bone
C: Osteoporosis
Anastrozole/exemstane
M: Aromatase inhibitors
C: PM women with ER+ breast cancer
Progestins
M: Decrease growth and increase vascularization of endometrium
C: OCPs, treat endometrial cancer, abnormal uterine bleeding
Mifepristone (RU-486)
M: Competitive inhibitor of progestins
C: Abortion - use with misoprostol (PGE1)
T: Heavy bleeding
OCPs
M: Estrogen and progestins inhibit LH/FSH and ovulation – progestins cause thick cervical mucus so sperm can’t enter and inhibit endometrial proliferation (no implantation)
T: Contraindicated in smokers > 35, history of DVT or stroke, or estrogen dependent tumors
Terbutaline, ritodrine
B2 agonsits that relax the uterus, decrease contraction frequency during labor
Danazol
M: Partial androgen agonist
C: Endometriosis
Testosterone, methyltestosterone
C: Hypogonadism and development of secondary sex characteristics, stimulation of anabolism
T: Masculinzation in females, inhibits LH release in men and causes gonadal atrophy, premature closure of epiphyseal plates
Finasteride
M: 5a reductase inhibitor
C: BPH and male pattern baldness
Flutamide
M: Competitive inhibitor at androgen receptors
C: Prostate cancer
Ketoconazole, Spironolactone
M: Inhibit steroid synthesis/steroid binding
C: PCOS
T: Gynecomastia, amenorrhea
