pharm 1 Flashcards
Concurrent administration of VPA and Lamotrigine can cause dangerous elevation of Lamotrigine levels via what mechanism?
VPA is a potent cytochrome P450 3A4 inhibotor, therefore decreasing metabolism of Lamotrigine.
What is the mechanism in which MAOIs cause hypertensive crisis?
MAOI-A is involved in the metabolism of tyramine in the gut. If you block this metabolism, tyramine can build up and cause hypertensive crisis.
All atypical antipsychotics block 5-HT 2A and Dopamine D2 receptors….
What is the only Atypical antipsychotic that also has the unique property of inhibiting both Serotonin AND Norepinephrine re-uptake.
Ziprasidone
All antipsychotics work by blocking DA receptors (antagonists). What is the only antipsychotic that also has partial dopamine receptor agonism?
Aripiprazole = partial D2 agonist.
All antipsychotic work on DA receptors. Abilify is unique in that it also works on? Which specific receptors and how?
- Partial agonist of 5-HT1A
2. Antagonist at 5-HT 2A
Antipsychotics can cause hyperprolactinemia via what mechanism?
DA blockade of the TUBEROINFUNDIBULAR TRACT
What neurotransmitters does Ziprasidone work on and how?
AGONIST - 5-HT 1A
ANTAGONIST- 5-HT 1D, 2A, and 2C. D2 and D3.
INHIBITS REUPTAKE - NE and Serotonin.
What three anti-depressants can be used for treatment of gastric ulcer, because of their high anti-histamine effects?
- Amitriptyline
- Doxepin
- Trimipramine
RESERPINE, is an antipsychotic not available in the US, and no longer used to treat psychosis, that is used to treat HTN and dystonia.
What is the MOA and how does this MOA cause SE of depression?
Reserpine interferes with Mg and ATP-dependend uptake of biogenic amines, which results in depletion of DA, NE, and 5-HT in the synapse, which causes depression.
Which TCA is MOST likely to cause Parkinsonism?
Amoxapine
It’s metabolite has DA blocking activity so causes EPS, can even cause NMS
