PFT 1 Flashcards
What are obstructive pattern D/O?
COPD(chronic bronchitis or emphysema)
Asthma
Cystic Fibrosis
Bronchiectasis
Obstructive patterns are inspiratory or expiratory and have what effect on lung volumes
Expiratory
Lung Vol. Increase or remain normal
Restrictive patterns are inspiratory or expiratory, and what happens to lung volumes?
Inspiratory
Lung volumes decrease
Hows does compliance differ in restrictive vs obstructive?
Compliance is high in obstructive and low in restrictive
What are Restrictive pattern D/O?
•Diseases that limit lung expansion by decreasing lung compliance:
sarcoidosis, pneumoconiosis, idiopathic/interstitial pulmonary fibrosis
•Loss of Lung Volume:
lung resection, pneumothorax, pleural effusion
•Chest Wall Deformity/Disease:
pectus excavatum, kyphosis, scoliosis, straight-back syndrome, neuromuscular disorders
(MD, ALS)
•Extra-thoracic Compression:
obesity, pregnancy, ascites
•Intra-thoracic Compression:
pulmonary tumor
What are the relative CI for PFT?
- infectious control
- high myocardial demand
- high sinus or inner ear pressure
- high intraocular pressure
- high intraabdominal pressure
What are some other CI for PFT?
- unwilling
- too young <5-6 yo
- ARS
- physically unable
What are the 3 main components of PFT?
- Lung volumes
- Diffusion capacity
- Flow rate
National Asthma Education Program and American Thoracic Society reccomend spirometry in dx what two conditions respectively?
Asthma
COPD
What shoud be generally avoided before PFT?
Smoking or other like it 1hr prior
xs exercise 1hr prior
intoxication 8hr prior
What is the predicted value based off of ?
norms based on Ht, gender, and Age
What are the American Thorasic Society guidelines for pt pre PFT?
- No smoking within 1 hr of PFT-
- Avoid vigorous exercise within 30 min of testing
- No eating a large meal within 2 hr of testing
- No bronchodilator meds within several hours of the PFT
What is considered good pt effort?
quick start
no coughing 1st sec
6 sec blown adults and 3 sec peds
How many acceptable trials for PFT are required, and how far apart can they be?
3 trials and 200 ml apart
