repetitive behaviour or mental acts eg repeating a phrase in your head to reduce anxiety
RF OCD
FH, age 10-20, pregnancy or postnatal, history of abuse, bullying, neglect
what scale is used for ocd
Y-BOCS
what is considered severe OCD
a person spends 3+ hours on obsessions/ compulsions and has severe interference with QOL, no control
1st mx mild funcional impairment OCD
mild CBT and exposure response
2nd mx mild funcional impairment OCD
SSRI or more intense CBT
1st line mx moderate OCD
SSRI (or clomipramine if they prefer) and intensive CBT
SSRI for body dysmorphia
fluoxetine
mx severe OCD
refer to pysch - while waiting offer SSRI + CBT
if SSRI effective in OCD how long should it be continued
12 months
dose of SSRI for OCD
higher and longer (12 weeks minimum) than depression
cluster A personality disorder
odd + eccentric: paranoid, shizoid, schizotypal
cluster B personality disorders
dramatic, emotional, erratic: antisocial, borderline (emotionally unstable), histrionic, narcissistic
cluster C personality disorder
anxious + fearful: obsessive, avoidant, dependent
