What are the cluster a personality disorders?
the odd/eccentric: schizoid, schizotypal, and paranoid
schizoid personality
- eccentric d/t affective detachment and emotional coldness
- socially isolated by choice
- may function well in some jobs but lead very solitary lives
- rarely have subjective distress or interpersonal chaos
- may look like prodrome/negative sx of schizophrenia
schizotypal personality
- flamboyantly eccentric
- oddities of thinking/speech, unusual ideas, constricted or inappropriate affect
- socially isolated (may be chosen, or due to eccentricities)
- may be at higher risk of becoming psychotic under stress
paranoid personality disorder
- have paranoid thoughts
- paranoia not of delusional proportions
- also more likely to become deulsional/psychotic at times of stress
cluster b personality
dramatic, emotional, erratic- think borderline, narcissistic, antisocial
Borderline
- lack of sense of self (ie genuine abscence of whole, integrated self-image)
- use maladaptive defense mechanisms (splitting, projection, projective identification)
- pts have chaotic interpersonal relationships, identity disturbance, sensitivity to abandonment, emptiness, affective instability, impulsiveness, repeated SIB, transient psychotic sx
cluster b personality
dramatic, emotional, erratic- think borderline, narcissistic, antisocial, histrionic
Borderline
- lack of sense of self (ie genuine abscence of whole, integrated self-image)
- use maladaptive defense mechanisms (splitting, projection, projective identification)
- pts have chaotic interpersonal relationships, identity disturbance, sensitivity to abandonment, emptiness, affective instability, impulsiveness, repeated SIB, transient psychotic sx
features of narcissistic personality disorder
- grandiosity of thoughts/behavior
- need for recognition of perceived specialness
- lack of empathy
- many meet criteria for other personality disorders, esp. borderline (but may be more functional due to “shell” of grandiosity)
histrionic personality disorder
- excessive emotionality and attention seeking
- despite striking displays of affect, often superficial quality to the relationships
- may be suggestive, impulsive, flirtatious/sexually provocative,
antisocial personality disorder
pervasive disregard for and violation of rights of others
- often seen in incarcerated men, often with substance abuse disorders
- external blame, no remorse, chronic lying/law-breaking, aggressive.
- must show conduct disorder before age 15
- May be charming/charismatic if it meets their immediate needs

cluster c
anxious/fearful: avoidant, dependent, obsessive compulsive (passive aggressive)
avoidant personality
social inhibition d/t feelings of inadequacy
- overlaps with social phobia
- pts wish they were more socially connected
obsessive compulsive personality disorder
- preoccupation w/details, order, perfectionism
- obsessions occur at the expense of felxibility, openness, and efficiency (often impairs ability to complete a task, impairs relationships)
