How many permit to works are there and what are they?
A permit to work is raised when a health risk is applicable to the work being carried out and there is not a SOP, this could include:
turning shaft
confined spaces
diving
embarking/disembarking hazardous fluids
shore supply cables
hot work
generic PTW (aloft)
safe to crane
What BR does permit to works fall under
BR167 chapter 7
What assessment to permit to works require
A consideration of a risk assessment to be formally documented on the PTW
What are the steps to carry out work in a confined space?
PTW and risk assessment raised
All fluids removed from tank/space
Ventilation inserted to confined space for 24h
Air monitored with gas free monitor
Gas free certification produced and attached to PTW
EEBD must be worn in space with standby EDBAs and sentry
Rescue team on standby
PPE to be worn accordingly
Requirements for diving
Safe to dive certificate
Underwater shiphaz/tag-out
Adjacent ships within 100m to carry out diving procedures
Process for bringing on shore supply cables
Permit to work
rack out ss breaker
test ss box dead
obtain castel key
ensure phases correct
attach earth cables
hand back castel key and check phase rotation (light on right)
parallel fwd and aft
