Intermittent claudification in peripheral vascular disease

400mg 2 - 3 times daily
Mechanism of pentoxiffyline
A xanthine derivative which is a nonselective PDE inhibitor
improves red blood cell flexibility
reduces blood viscosity
decreases platelet aggregation
antagonist at adenosine 2 receptors
Contraindications of pentoxiffyline
Cérébral hemorrhage
Retinal hemorrhage
Severe cardiac arrhythmias
Hypersensitivity
Acute MI
Pentoxiffyline in lactation
Excreted in breast milk, do not use
Pentoxiffyline in pregnancy
Category C, use when benefits outweigh risks
Adverse effects of pentoxiffyline
Nausea and vomiting
GIT disturbances
Angina
Pancytopenia
Hypotension
Angioedema
Bronchospasm
Hemorrhage
Headache
When is dose adjusted for pentoxiffyline
Hepatic impairment
Renal impairment, eGFR<30: give 400mg once daily
Digestive or CNS side effects: Decrease to 400 mg BID
