Peripheral Nerve Disease Flashcards
what is the flow from motor neuron to muscle fiber?
motor neuron- peripheral nerve- neuromuscular junction- muscle fiber
Where do central nerves go?
in brain and spinal cord
Pripheral nerves
spinal cords to hands, arms legs and feet
autonomic nerves
from spinal cord to the lungs heart stomach bladder and sex organs
What is the #1 cause of focal neuropathies?
trauma, #2 is entrapment
poly neuropathy- glove stocking pattern
know this
Diabetic neuropathies
Distal symmetric sensorimotor
Autonomic [20%; HR, GI/GU, respiratory, pupil]
Painful distal neuropathy with weight loss
Large fiber, ataxic
Neuropathy with glucose intolerance
Diabetic amyotrophy (proximal neuropathy)
Diabetic truncal neuropathy
Cranial neuropathy
Tx of diabetic neuropathies:
Tight glycemic control Orthostatic hypotension --Lifestyle changes --Fludrocortisone, DDAVP, midodrine ED Pain treatment (tramadol, oxycodone, gabapentin)
Trigeminal neuropathty
adult disabling, lacerating facial pain V2 or V3 distribution Sudden onset paroxysmal unilateral pain not during sleep provocable (talking, chewing and shaving)
tx of trigeminal neuropathy:
anticonvulsants (carbazepine)
microvascular decompression
Bell’s Palsy:
dull ache behind left ear female inability to smile eyelid cannot close cannot wrinkle forehead dry eye decreased tasted sensation
tx of bells palsy:
oral prednisone
Meralgia Paresthetica
lateral femoral cutaneous nerve (L2-L3)
compressed of stretched in obese or diabetic pts
pain at thigh unilaterally, relieved by sitting
hydrocortisone to treat
