etiology of venous thrombosis
virchow’s triad- venous stasis; damage to endothelium; hypercoagulability of blood
venous thromboembolism
deep veins of arms/legs, pelvis, vena cava, and pulmonary system
manifestations of VTE
lower extremities: unilateral edema; pain; tenderness with palpation; dilated superficial veins; full sensation in thigh/calf; paresthesia; red, warm, > 100.4 degrees
complications of VTE
pulmonary embolism; s/s; management
nursing management: preventions for venous thrombosis
avoid oral contraceptives, drink adequate fluids; exercise legs during long periods of bedrest/sitting
inpatient preventions for venous thrombosis
leg exercises, early ambulation; SQ heparin, lovonox; SCDs; venous plexus foot pump, graduated compression stockings; active clots: bedrest and ambulation
drug therapy for venous thrombosis
heparin
surgical treatment of vein thrombosis
open venous thrombectomy; inferior vena cava interruption devices; inferior vena cava filtration
open venous thrombectomy
incision into vein to remove clot
inferior vena cava interruption devices
filters placed via right femoral internal jugular veins to trap clots without impeding blood flow
VTE risk factors
smoking, hormone therapy, travel, prolonged sitting, s/s of PE
ambulatory care for venous thrombosis (management)
drug doses, actions, side effects, and routine blood tests; avoid falls and trauma; apply pressure to bleeding sites for 10-15 minutes; no ASA/NSAIDs; limit alcohol
varicose veins
dilated (less than or equal to 3 mm in diameter), toruous superficial veins
