General characteristics of use of perioperative antibiotics?
- Prophylaxis or therapeutic
- Selection based on expected flora
- Will reduce bacteria numbers
- Required due to low immune defences after surgery
When would you not use antibiotics?
During clear or clean-contaminated surgeries:
- ASA status I, II or III
- Short (<90 minutes)
- No implants
When would you use prophylactic antibiotics?
- 30-60 minutes before incision
- clean or contaminated surgeries lasting >90 minutes
- when an implant is placed
- risky surgery, e.g. neuro
- contaminated wounds
- ASA status III, IV or V
When do you use antimicrobial therapy?
- Dirty surgeries
- Pre-existing infection
