What is the perineum?
Area of the body inferior to the pelvic diaphragm and within the pelvic outlet
What is contained in the perineum?
The external genitalia, terminal portions of the urogenital system and GI tract
What makes up the border of the perineum?
Anterior: pubic arch
Posterior: tip of coccyx
Lateral: inferior rami of pubis and ischium/sacral tubal ligaments
Perineal body
The midpoint of the transverse line that goes through the central point of the perineum (where muscles meet, important support)
What is the superficial fatty layer continuous with?
The camper’s fascia in the abdomen
The superficial fascia in the thigh and the superficial anal fascia
Where does the superficial fatty layer continue in the female?
into the labia majora and inserts into the perineal body
What happens to the superficial layer in the male?
Diminishes in the UG triangle but continues around the penis and scrotum to be replaced by the dartos muscle ( a smooth muscle)
what forms the dartos fascia in males?
The deep layer of the superficial fatty
How is the superficial perineal pouch formed?
The superficial perineal fascia attaches to the posterior border of the perineal membrane
Buck’s fascia
A deep fascial layer that is prolonged over erectile bodies of the penis in males
Investing layer
Deep perineal fascia – surrounds the muscles in the deep perineal pouch (ischiocavernosus, bulbospongiosus, superficial transverse perineal)
What is the perineal membrane
A thin sheet of deep fascia that is attached to the ischiopubic rami laterally, but does not reach the pubis anterior.
It spirals to the bladder neck – its not just a flat sheet
