Pericarditis Flashcards
Symptoms of Pericarditis
Acute, Sharp pain
Pleuritic pain: aggravated by movement, exercise and swallowing
Relieved by leaning forwards
What can you hear on auscultation?
Pericardial rub
Rough scratching sound- 3 components: systolic, diastolic and atrial-systolic
How can the pericardial rub murmur be accentuated?
By leaning the patient forwards
In acute pericarditis, what does the ECG show?
Saddle shaped ST elevation
Widespread and generalised- all 12 leads
In acute pericarditis, what does the x-ray show?
‘Pericardial effusion’- accumulation of fluid in the pericardial cavity- enlargement
Causes of Pericarditis
Idiopathic Inflammatory Infectious Post surgical/ trauma Pulmonary arterial hypertension Radiotherapy Malignancy Metabolic
Inflammatory causes of Pericarditis
Post MI- Dressler syndrome
Connective Tissue Disorders
Infectious causes of Pericarditis
Coxsackie Virus A/B Varicella Herpes simplex Influenza virus Echovirus TB Bacterial
Malignant causes
Primary: pericardial mesothelioma
Metastatic
Lymphomas
Metabolic causes
Hypothyroidism
Uraemia
Complications of pericarditis
Pericardial effusion- symptoms when large
Cardiac Tamponade
Management of Cardiac Tamponade
Pericardiocentesis
What is cardiac shock?
Low CO
Cerebral confusion
Severe hypotension
What causes cardiac shock?
Pericardial cavity fills with fluid which causes the pericardial pressure to increase above the filling pressures of the heart which prevents cardiac filling
