Squamous cell carcinoma pathology
> 90% of the cases
Spread is local, lymphatic (inguinal first)
Haematogenous spread to lungs.
Etiology
Presence on foreskin➡️ SMEGMA aka poor hygiene➡️ chronic irritation➡️ carcinoma
HPV 16 & 18
Pre malignant conditions: CIS, erythroplasia of queyrat, Bowen disease, cutaneous penile horn
Pathology
Squamous cell= most common >90%
ulcerating with a rolled edge OR
exophytic cauliflower-like
Verrucous ca. Extensive penile warts
Management
Pre-malignant
CIS: 5 fluoro uracil
BXO: circumcision, local excision of lesion
Leukoplakia: local excision
Nb‼️all surgery must’ve done under AB cover.
Primary lesion
Depends on where it is. @foreskin circumcision
@glans or distal shaft: partial penectomy
@prox shaft: total penectomy & perineal urethrostomy
