What is detrusor instability?
How do you treat it?
- Hypertonic, overactive, motor urge incontinence. Spasms of the detrusor.
- The detrusor constricts the bladder.
- Oxybutnin (antispasmotic)
What is the cause of overflow incontinence?
How do you treat it?
Absent detrusor contractions. Pts have pressure, fullness and frequency with little urinary output.
- Seen in MS, diabetes, Trauma, Anti-spasm meds.
- bethanichol
What is functional incontinence?
When a person feels the urge, but for one or more physical or mental reasons they are unable to get to a bathroom.
What are the risk factors for pelvic organ prolapse?
Parity, age, obesity, connective tissue disorders, chronic constipation.
NOT Hysterectomy
What is the surgical option for stress incontinence?
Retro pubic Urethroplexy
What is intrinsic sphincteric deficiency and how do you treat it?
no activity of the urethral sphincter. Treat with urethral bulking procedure.
What is the next step in managing a patient who has uterine prolapse?
See if they are symptomatic or not.
If asymptomatic, dont do hysterectomy.
What are the surgical treatments for vaginal prolapse?
Colpocleisis- Done for non sexually active pts
Sacrocolpoplexy (anchors vagina to sacrum)- done for younger pts.
What are anterior and posterior repairs for prolapse?
Help strengthen the vaginal wall to prevent cysto/rectocele
