0 - 2 months of age
meningitis:
group b strep
listeria
ecoli
signs are usually not usually there (nucchal rigidity)
emperic treatment:
use ampicillan and cefotaxime
or ampicillan and amino glycoside
and dexamethasone (steroid)
after 2 months of age
meningitis:
strep pneumonia
neisseria
h. influenza
emperic treatment:
use ceftriaxone and vancomycin
and dexamethasone (steroid)
pertussis
cbc will show lymphocyte high count (unlike most bacterial infections that are neutrophils) very high
Rx: is supportive, give erythromycin prevents patient from giving it to someone else, does not help the patient
treat household contacts
cat scratch
bartonella henselae lymphadenitis lasting > 3 weeks incubation 3 - 30 days unilateral conjuctivitis PCR of tissue is best test warthins starry silver stain rx: macrolides only if patient is not getting better
tuberculosis
intradermal ppd
four to eight weeks to turn positive
sputum cultures difficult for children
early morning stomach aspirate three days in a row
infants most likely to have symptoms
TB meningitis ** also give steroids!!
if mom has disease, then baby gets treated. they can be together.
lyme disease
borelia burgdorferi ixodes ticks carry the spirochete 1- erythema migraines 2- bells palsy and cardiac disease 3- arthritis in large joints
tx: doxycycline (avoid in children under 8)
for those you use amoxicillan
meningitis: ceftriaxone or penicillan
good prognosis if caught early
rocky mountain spotted fever
rickettsia rickettsi tick exposure southeast (north carolina) rash starts on extremities and moves centrally petechial that becomes purpuric serology for dx may have hyponatremia
tx: doxycycline always (don’t worry about staining teeth)
