AOM - Uncomplicated
without otorrhea
AOM - Non-Severe
presence of mild otalgia AND temp < 39 C
AOM - Severe
presence of moderate-to-severe otalgia OR fever 39 C or greater
AOM - Recurrent
3 or more documented and separate occasions of AOM in the preceding 6 months
OR
4 or more episodes in the preceding 12 months with at least 1 episode in the last 6 months

AOM - Bacterial Causes
S. pneumoniae
H. influenzae
M. catarrhalis
AOM - Risk Factors
genetic predisposition
family history of recurrent AOM
allergies
lack of breastfeeding
low socioeconomic status
passive smoke exposure
daycare attendance
pacifier use
winter season
AOM - Signs & Symptoms
otalgia
irritability
headache
disturbed sleep
poor feeding
vomiting / diarrhea
fever
AOM - Physical Exam
erythematous tympanic membrane
fluid in the middle ear
AOM - Diagnostic Criteria
moderate-to-severe bulging of the tympanic membrane
OR
new onset of otorrhea not caused by otitis externa
OR
mild tympanic membrane bulging with recent onset of otalgia or erythema
AOM - Antibiotic Therapy Indications
6 months to 2 years: otorrhea with AOM, unilateral or bilateral AOM with severe symptoms, bilateral AOM without otorrhea
2 years or older: otorrhea with AOM, unilateral or bilateral AOM with severe symptoms
AOM - Watchful Waiting
6 months to 2 years: unilateral AOM without otorrhea
2 years or older: bilateral or unilateral AOM without otorrhea
AOM - First-Line Treatment (Initial)
amoxicillin 80-90 mg/kg/day, divided
OR
amoxicillin-clavulanate 90 mg/kg/day, divided
AOM - First-Line Treatment w/ PCN Allergy
cefdinir 14 mg/kg/day
OR
cefuroxime 30 mg/kg/day, divided
OR
cefpodoxime 10 mg/kg, divided
OR
ceftriaxone 50 mg IM
AOM - Treatment After 48-72 hr of Failure of Initial Treatment
amoxicillin-clavulanate 90 mg/kg/day, divided
OR
ceftriaxone 50 mg IM for 3 days
