What are 2 general reasons BP may increase in persons with renal injury?

A
  1. Increased fluid retention
  2. Vessel stenosis (kidneys getting less volume flow so they think there is a systemic hypotension and in turn activate RAAS)

Nephritic vs. Nephrotic?

A

Nephritic: Less than 3.5 g/day protein, moreso due to inflammation and GBM disruption. Will see Azotemia and oliguria because of decreased GFR, Hematuria, RBC casts

Nephritic: massive proteinuria, hypoalbuminemia, edema…All due to Podocyte disruption so large molecules can get through!

Describe the process of hematuria in HSP.

A

Immune complexes deposit in small vessels–> inflammation –>necrosis of kidney vessels –>leakage of blood into urine

A

Type III-Arthus (vasculitis due to immune complex deposition).
