What should you do if patient presents with an adverse reaction (non-life threatening)?
Make note on PMR, contact prescriber and report on yellow card scheme.
What is adverse reaction?
Serious bad reaction - even if expected
What is the NRLS system?
National reporting and learning system. An anonymous way of reporting patient safety incidents.
What should you do with near misses?
Make a near miss log for future learning
What should you do with dispensing errors?
Let patient know, apologise and tell them how you’ll make things right.

What should you do if child abuse is suspected?
Make record of what was witnessed.
Call 999 if necessary.

What is the legal age of Sexual consent and what is the Fraser guidance?
Person under 13 can’t consent to sex.
Sex with people under 16 is illegal.
Fraser guidance - gives criteria where you can give advice / contraceptives to those under 16
What is pharmaceutical care?
The use of pharmacists to get the best out of patient medicine whilst reducing risks
At what point should a Med Rec be done?
Ideally within 24hrs of hospital admission.
Anytime patient changes care setting - to hospital, to different ward, on discharge.
