What is a sentinel event?
An unexpected occurrence, requiring immediate investigation or response that result in an unanticipated
death, serious physical injury
eg, Abduction of individual receiving care, treatment or services
What criteria qualifies suicide as a sentinel event?
- Individual receiving care, treatment or service in an around the clock facility
- Within 72 hours of discharge
What are Sentinel events for infants?
- Discharge of an infant to the wrong family
- Unanticipated death of a full term infant
- Severe neonatal hyperbilirubinemia (bilirubin greater than 30 miligram/deciliter)
How is sexual assault a sentinel event?
If pt are sexually assaulted by a patient or staff member
What are Surgical Sentinel Events?
- Surgery on the wrong pt or body part
2. Unintended retention of a foreign object in an individual after surgery or other procedure
How is blood administration a sentinel event?
Hemolytic transfusion reaction involving blood or blood products having major incompatibility
How is radiation therapy considered a sentinel event?
Prolong fluoroscopy with cumulative dose greater than 1500 rads to a single field or delivery of radio therapy to wrong body region or greater than 25% above planned dose

Acronym for PASS when using a fire extinguisher?
- Pull the pin
- Aim nozzle at the base of the fire

What are alert wristbands?
colored wrist bands used in conjunction with standard identification wristband to alert staff to potential risks associated with routine pt care. eg allergy, blood, fall and flight
What is the national quality forum?
A non profit organization whose mission is to lead national collaboration to improve health and healthcare quality through measurement
National quality forum goal?
to eliminate serious, preventable and costly medical errors that should never happen
