Types of ID

A

3 forms of ID are required: usually a photo, name, address, DOB. If the patient cannot speak ID can be checked on a hospital wristband or by their passport if they don’t speak English.

Who checks ID?

A

ID should be identified by the Operator who will carry out the procedure/exposure. That person must have their signature clearly marked to confirm they carried out the exposure.

What else should be checked?

A

Anatomical site, laterality, pregnancy status, consent, previous imaging, modality, timing, fractionation.
