Patient Relationship Phases:
Pre-interaction
Orientation phase
Working phase
Termination phase
- Obtaining available information about the patient from chart and other health team members
- Initial assessment of patient
- Plan first meeting with patient
Pre-interaction Phase
- Establishment of trust and rapport
- Maintaining privacy
- Gathering essential information from the patient or relatives
- Formulate nursing diagnosis
Orientation phase
- Active problem solving
- Periods of intense effort and resistance/regression (ego defense mechanism due to stress)
Working phase
- Nurse and patient take on those activities that enable them to end the relationship in therapeutic manner
- Positive/negative feelings often accompany the end phase
Termination phase
A standardized survey tool to measure the patient’s perception of quality care provided during their experience while a patient is at an acute-care hospital
