part 8 Flashcards
The treatment for a pelvic fracture is based on the severity of the injury. What is the tx for a stable pelvic fracture?
- bed rest
- pelvic sling
- progressive ambulation
The treatment for a pelvic fracture is based on the severity of the injury. What is the tx for a complex/displaced (open book) pelvic fracture?
- emergent surgery
- external fixation
- ORIF
What is commonly found in a pt with a hop fx?
- common for older adults
- 95% from falls
What are the 2 types of hip fractures and what are they associated with?
- Intracapsular: (within joint capsule), associated with osteoporosis and minor trauma
- Extracapsular: (outside joint capsule) caused by severe direct trauma or a fall
What are the manifestations and S/S of a hip fracture?
-displaced femoral head causes disruption of blood supply and avascular necrosis
-S/S:
external rotation
muscle spasm
shortening
pain
tenderness
- temporary until surgery (24 to 48 H)
- relieves muscle spasm
bucks traction
What are the 3 types of surgical repairs of a hip fx?
- repair with internal fixation device (screw)
- partial replacement (replacement of femoral head only)
- total hip replacement (femur and acetabulum)
Explain pre-op hip fx surgery.
-current/past med hx pain management: -analgesic -muscle relaxer -positioning -traction -teaching
Explain post-op hip fx surgery.
- VS
- I&O
- respiratory function (TCDB)
- pain control
- S/S of bleeding
- neurovascular assessment
- approved position changes
What are some post op rules to prevent dislocation after hip fx surgery?
-activities increase dislocation
>90 degree flexion
-adduction across midline
-internal hip rotation
