Your patient is a 51 year old obese male with a history of heavy alcohol intake and smoking. He is complaining of acute onset deep epigastric pain that radiates to the back. The pain is worse when he is supine or walking, and is improved by sitting and leaning forward. His serum amylase and lipase are elevated.
