Nursing role during regional anesthesia list:
Epidural
Spinal
Combined
Epidural will take longer.
Major side effect is Hypotension.Can give ephedrine.
Spinal and combined can have head aches & can travel upward to effect breathing.
Give a fluid bolus with them.
C shape is needed. Either laying down or sitting.
The spinal is given in section and they must sit up.
Vaginal delivery is given epidural.
List :
Patient refuses
Patient has infection near spine
ICP is present
Thrombocytopenia or not enough platelets
Anticoagulant therapy hasn’t been tapered off
Allergies to the anesthesia
Tumor at the site
Tattoo ink over site
Abnormalities or injuries like spina bifida
Nursing role:
Fluid bolus
Vitals every 5 min with machine
Make sure resuscitation equipment works
Make sure patient emptied their bladder
Assist w positioning
Stay with patient for 30 minutes
Advocate for patient
Name the systemic analgesia meds or the opioids/narcotics

Nubain
Demerol
Fentanyl
The antidote is Narcan.
Stadol and Nubain have the ceiling effect.
Stadol and Nubain can cause withdrawals.
Demerol is not used anymore.
Fentanyl is the powerful one.
Respiratory depression especially if given too late.
All of these meds can affect mom and baby.

When the uterus must be relaxed quickly

Big side effect is fetal depression.
The uterus not clamping
Not remembering the birth
Aspiration risk
Bicitra antacid
Reglan to get them to empty stomach
Tagemet, Pepcid, Zantac to decrease juices
