The most important thing in this lady is adequate analgesia with the aim of early mobillisation, to encourage physioterapy participation and also to maintain her psychosocial wellbeing in the the setting of her history of chronic severe pain causing depression and also bearing in mind that she has a chronic long standing use of opioid analgesia.
However, there are conflicting issues here with the increased threshold of analgesia tolerance, drowsiness and respiratory depression.
In term of pain management, I’d consider using NSAID/COX inhibitor which is useful in this setting, I wouuld also optimise the opioids, pregabalin and engage the acute pain services.
