Atrial fibrillation
A
For most patient DOAC is preferable over vitamin K antagonist (warfarin)
Warfarin is preferable in
- ESRF ( creatinine clearance < 25)
-Rheumatic heart disease with MV stenosis with valve area< 1.5cm
-mechanical heart valve
Edoxaban- generally not used in ppl with very good creatinine clearance > 95 as it’s mostly renally cleared
If Creat cl between 50-95 ——> 60 mg daily
Creat cl between 15-50——-> 30mg daily
Avoid in Creat clearance < 15
>65 yo —-> give 30mg
Similarly if weigh <60kg
Or ppl using verapamil and quinidine ( potent p glycoprotein inhibitor)

Lipid target
A
LDL-c <1.8 mmol/ L for secondary prevention
LDL-c < 2.0 in primary prevention
Total Cholesterol < 5
