Fentanyl (Pain)
A
- IV/IO: 1-3 mcg/kg (max single dose 200 mcg) q 5-10min
- IN: 1-3 mcg/kg q 5-10 min
- IM: 100 mcg q 1 hr PRN
- Continuous infusion: 25-300 mcg/hr
Mix 300 mcg in 100 mL of
NS (3mcg/ml)

Morphine (Pain)
A
- IV/IO: 2-5 mg q 15-20 min
- IM: 2-10 mg once for pain

Ketamine (Pain)
A
- IV/IO/IM/IN: 0.15-0.3 mg/kg q 5-10 min
- Consider Atropine 0.4 mg IV for hypersecretion
- Continuous infusion: 0.1-0.2 mg/kg/hr (Transports > 30 mins)
*Mix 500 mg in 100 mL NS (5 mg/ml)

Tylenol (Pain)
A
1000 mg IV over 15 min once. May use for mild/moderate pain

A
IO: Lidocaine 2% (cardiac bristojet) 20–40 mg IO Slowly over 2-3 minutes

A
Intradermal: 0.2 mL or less of 2% lidocaine
