PAEDS MANAGEMENT OF COMPROMISED 6'S Flashcards
What is the ideal time to extract, FPM, if you are extracting it?
8.5-10.5 years old.
What does the ideal age for PFM extraction coincide with?
Commencement of calcification of the bifurcation of the 2nd molar
Timing of extraction of PFM is more critical in: Mandible/Maxilla?
Mandible
If space closure is encouraged should you extract earlier or later in the ideal age period for extraction of FPM?
Early, towards 8.5 rather than 10.5
What will happen if 6’s are extraction before 8 years?
5 will drift distally and become rotated
Extraction of 6’s after 12 will result in what?
Excessive spacing, tilting, rotation of teeth, esp 5 and 7.
If you are encouraging space closure of the 7 after extraction of the 6 what can you do about the opposing teeth?
Use a night time removable appliance to prevent overeruption of opposing dentition into space until the space closure is achieved.
What are balancing extractions?
Removal of a contra lateral tooth, which doesnt have to be a FPM to maintain the dental midline
What are compensating extractions?
Extraction of an antagonist molar to prevent its over eruption
When should balancing and compensating extractions be considered?
During the mixed dentition phase if no active appliance tx is to be undertaken
What are the consequences of lost upper FPM?
Maxillary molars develop with distal angulation
Never extract maxillary FPM in what class patients?
Class 3
When molars are developing maxillary molars develop with a ____ angulations and mandibular molars develop with a ______ angulation
a. Distal
b. Mesial
What are the consequences of lost lower FPM?
Mandibular molars develop with mesial angulation. Mesial tilting and lingual rolling of the 7s.
