What is cold shock?
What does it respond well to?
- low cardiac output + high systemic vascular resistance
- cold, pale, mottled
- prolonged capillary refill time
. - fluids + ionotropes (adrenaline)
Presentations of paediatric sepsis
- pale/mottle skin +/- non blanching rash
- appears ill
- tachypneoa or grunting
- tachycardia
- no response to social cues
- weak, high pitched or continuous cry
- fever or very cold
What is SEPSIS 6 in children?
- O2
- take bloods
- IV or IO antibitoics
- fluid bolus of crystalloids (0.9% sodium chloride) + maintenance
- escalation
- ionotropes
What is hot shock?
What does it respond well to?
- high/normal cardiac output + low systemic vascular resistance
- warm peripheries, flushed + bounding pulse
- fast capillary refill
. - fluid + vasopressors (noradrenaline)
When should you suspect sepsis in a child?
Evidence of infection + systemic inflammatory response
What antimicrobial is given in sepsis if child is under 1 months old?
What is the likely organism?
- Cefotaxime: group B strep, strep pneumoniae + staph aureus
- Amoxicillin: listeria
- Gentamicin: E. coli
- Aciclovir: herpes simplex virus
What antimicrobial is given in sepsis if child is between 1 - 3 months old?
What is the likely organism?
- Ceftriaxone: group B strep, strep pneumoniae, staph aureus
- Amoxicillin: listeria
What antimicrobial is given in sepsis if child is over 3 months old?
What is the likely organism?
Ceftriaxone
- strep pneumoniae
- neisseria meningitidis
- staph aureus
If you cant gain IV access what can be done instead?
IO

Treatment of cold shock vs warm shock
- cold shock: fluids + ionotropes e.g. adrenaline
- warm shock: fluids + vasopressors e.g. noradrenaline
