Traffic Light System
Colour- do they look more pale to parent/carer
Activity- playing and interacting as usual or more tired
Resp- any difficulty with breathing
Hydration/circulation- drinking enough, are they having as many wet nappies as usual
Other- hot, recorder temp, any rashes
Pregnancy and birth history
Prenatal
- maternal illness eg. DM, pre-eclampsia, infections
- did mother smoke, take alcohol, drugs etc.
- complications of pregnancy and labour
Natal
- normal delivery vs ceasean
- did pregnancy reach term
- birth weight
Postnatal
- Was child well
- Did they require admission
Feeding history (infants and toddlers)
Bottle/breast- which one and details
Weaning- at age were solids introduced, any difficulty weaning
Feeding pattern- what do they eat, any trouble with certain foods eg. dairy
Developmental history
Concerns about development
Milestones- have they met them yet
NB- ask for red book/are community midwives happy
Immunisation history
Up to date
When was the last one/ what for
NB- adapt the paediatric history for the age and circumstances of the child eg. feeding and postnatal history not required for adolescent with new cough
