I’ve performed a cardiac exam on Mr x a x yo patient.
General inspection: calm and comfortable at rest.
Paraphernalia around the bed
Pulse was x , character
BP was x
Hands and Face - no peripheral stigmata of CVD
Carotid pulse normal
Chest - no scars or chest wall deformities
Auscultation - H1 + 2 present
Lung bases clear
No peripheral or sacral oedema
Contraindications to warfarin?
Advice?
- bleeding diathesis
- peptic ulcer disease
- risk of falls
Advice
- medic alert bracelet
- avoid certain foods e.g. grapefruit/ Cranberry juice (increases effects)
- come to hospital if uncontrolled bleeding
How do you describe nystagmus?
Nystagmus is slow drift of eyes in one direction and fast correction. this fast phase is defined as the direction of nystagmus.
Peripheral problems - nystagmus away from side of lesion
Central problems (cerebella/ brainstem) - nystagmus towards side of lesion
