What do pacemakers do?
Deliver controlled electrical impulses to specific areas of the heart to restore to normal electrical activity
Where is pacemaker implanted?
Under the skin (Left anterior descending wall) or axilla, and wires implanted in relevant chambers
Contraindications for older pacemakers?
Don’t do MRIs
Indications for a pacemaker?
Symptomatic bradycardias Mobitz 2 3rd degree heart block severe HF Hypertrophic obstructive cardiomyopathy
Single chamber pacemakers?
Leads are in single chamber (RA/RV)
If the SA node is the issue where is the lead of a pacemaker placed?
RA
If AV conduction is abnormal where is the lead placed?
RV
Dual chamber pacemaker?
Leads in both RA and RV so it can synchronize the chambers contractions
Triple chambers are also called?
Biventricular
Where are the leads in triple chambers?
RA, RV, LV
What are ICDs?
Implantable cardioverter defibrillators
-Continually monitor heart & apply defibrillator shock to cardiovert the patient back to sinus rhythm
What does an ECG look like when patient has a pacemaker?
- Sharp ventrical line on all leads
- Before each P wave= lead in atria
- Before each QRS complex = lead in ventricle
