Causes of serotonin syndrome
- MAOi’s
- SSRI’s interacting with St John’s wort or Tramadol
- Ecstasy
- Amphetamines
Features of serotonin syndrome
- Neuromuscular excitation (Hyperreflexia, myoclonus and rigidity)
- Autonomic excitation (Hyperthermia/Sweating)
- Altered mental state (confusion)
Management of serotonin syndrome
- Supportive with IV fluids
- Can give benzos
- Severe cases = cyproheptadine and chlorpromazine
What are some features of opioid misuse
- Rhinorrhoea
- Needle track marks
- Pinpoint pupils
- Drowsiness
- Watering eyes
- Yawning
What are some complications of opioid misuse
- Viral infection secondary to sharing needles - HIV, Hep B/C
- Bacterial infection secondary to injection - infective endocarditis, septic arthritis, septicaemia, nec fasc
- VTE
- OD can lead to resp depression and death
- Psych problems e.g. craving
- Social problems e.g. crime, prostitution, homelessness
Emergency management of opioid OD
IM/IV naloxone - has a rapid onset and relatively short duration of onset
Harm reduction intervention of opioid use
- Needle exchange
- Offer testing for HIV and Hep B/C
Management of opioid dependence
These patients are usually managed by specialist drug dependence clinics and patients may be offered maintenance therapy or detox
Detox:
Methadone or Buprenorphine
How do we monitor compliance to opioid detox and maintenance
urinalysis
detox takes 4 weeks as an in patient and 12 weeks in the community
Features of digoxin toxicity
Generally unwell, lethargy, N&V
ANOREXIA, CONFUSION and YELLOW-GREEN VISION
Arrhythmia’s
Gynaecomastia
Precipitating factors or digoxin toxicity
- HYPOKALAEMIA
- Increasing age
- Renal failure!!!
- MI
- Electrolyte abnormalities - low Mg/Albumin and high Ca/Na
- Hypothermia
- Hypothyroid
- Drugs
Which drugs precipitate digoxin toxicity
- Amiodarone
- Verapamil
- Diltiazem
- Ciclosporin
- Drugs causing hypokalaemia - thiazides and loop diuretics
Management of digoxin toxicity
Digibind
Correct any arrhythmia
and Monitor the K
