Outcome identification
Purposes of Outcome Identification:
- Provide individualized care
- Promote patient participation
- Plan realistic and measurable care
- Allow involvement of support

A choice that comes first over other possible choices; based on urgency or importance.
Priority
Ranks the order of nursing diagnoses in terms of importance to the patient.
Priority setting

High priority
Problems resulting in unhealthy consequences but not likely to threaten life.

An educated guess, made as a broad statement about what that patient’s state will be after the nursing intervention is completed.
Patient outcome
Directly addresses the problem stated in the nursing diagnosis. Must be behavioral; written to indicate a desired state.
Patient outcome
Patient outcomes contain an:
action verb and qualifier
It can be met in a relatively short period; several days or a week.
Short-term outcome
Requires more time; several weeks or months.
Long-term outcome
Measurable behavior and perception demonstrated by an individual, family, group or a community that is responsive to a nursing intervention.
Nursing outcome
A system that can be used to select outcome measure related to a nursing diagnosis.

States what the nurses will do.
