The judgment of the effectiveness of nursing care to meet patient goals based on patient’s responses.
Evaluation phase
A planned, ongoing, purposeful activity that determine the client’s progress; the step of the nursing
process in which the nurse evaluates the results of a nursing action.

Plan of care
Evaluation involves determining:
outcome attainment
Purpose of Evaluation:
- Examine patient’s responses to nursing interventions
- Compare patient’s behavioral responses to outcome criteria
- Appraise goal attainment or problem resolution
- Provide basis for revision of plan of care
- Appraise the nursing care delivered
- Monitor the quality of nursing care
Evaluation skills:
- Knowledge of standard of care
- Knowledge of normal patient responses
- Conceptual models and theories of nursing
- Ability to monitor the effectiveness of nursing intervention
- Awareness of clinical research
Types of Evaluation:
- Structure evaluation
- Process evaluation
- Outcome evaluation
Focuses on the attributes of the setting or surrounding; deals with environmental aspects that influence the quality of care.
Structure evaluation
Examples of Structure evaluation:
- Availability of equipment
- Layout of physical facilities
Focuses on the nurse’s performance and whether the nursing care provided was appropriate and competent.
Process evaluation
Evaluation on the patient and the patient’s function. Determines the extent to which the patient’s behavioral response to nursing intervention reflects the desired patient goal and outcome criteria.
Outcome evaluation
Activities of Evaluation:
- Review patient goals and outcome criteria
- Collect data
- Measure goal/Outcome achievement
- Record judgment or measurement of goal attainment
- Revise or modify the plan of care
Patient response occurred that is the actual response is the same as the desired outcome.
Goal met
Some, but not all, desired behaviors were observed, or the predicted outcome is achieved only part of the time.
Goal was partially met
The desired client response did not occur by the target time, or the actual outcome does not match the desired outcome.
