What?
Infection of middle ear from URT through Eustachian tube
Pathogens most commonly associated with URT infections?
Respiratory synctial virus
Strep. pneumoniae
H. influenza
Moraxella Catarrhalis
Who?
Children under 10
Acute or chronic?
Can be acute or chronic
What does infection cause?
Inflammation of middle ear mucosa & inflammatory exudate in middle ear space
Clinical features?
Otalgia
Hearing disturbances
Typical presentation?
Otalgia followed by discharge
If it doesn’t resolve what can happen?
Tympanic membrane perforation & discharge
Treatment?
NSAIDs
Often viral & will settle in 72 hrs without antibiotics
If systemic features to after 72 hours - systemic antibiotic
Which systemic antibiotic is used ?
Amoxicillin
In what age group is systemic antibiotic especially important in?
Children < 2 years
If tenderness over mastoid?
Urgent ENT referral
