Caused by Rickettsia rickettsii (obligate intracellular–can’t synthesize ATP) through a tick vector

Rocky Mountain Spotted Fever Presentation
Headache fever, rash (vasculitis)
The rash typically starts at WRISTS and ankles and then spreads to trunk, palms, and soles
Palms and soles rash also seen in Coxackie A virus and secondary syphilis

R. typhi: endemic (fleas)
R/ prowazekii: epidemic (human body louse)
Rash starts centrally and spreads out, SPARING palms and soles

Ehrlichia. Tick vector
Monocytes with morulae (berry like inclusions) in cytoplasm
Rash rare
Anaplasmosis Cause, Vector, and Presentation
Anaplasma. Tick vector
Granulocytes with morulae in cytoplasm

Q Fever Cause and Transmission
Coxiella burnetii. No arthropod vector (tick feces and cattle placenta release spores that are inhaled)
Presents as pneumonia
Chlamydiae Basic Info and Lab Diagnosis
Cannot make their own ATP
Obligate intracellular
Have elementary (infectious) and reticulate (replicate) bodies
Diagnosis: Cytoplasmic inclusions seen on Giemsa or fluorescent antibody stained smear
Chalmydiae Types and Presentations
Chlamydia trachomatis is an STD that causes reactive arthritis (Reiter syndrome), follicular conjunctivitis, urethritis, and PID
C. pneumoniae and C. psittaci (avian reservoir) cause atypical pnuemonia and are transmitted by aerosol
Chalmydiae Treatment
Azithromycin or doxycycline
Chlamydia trachomatis serotypes
A-C: Chronic infection, cause blindness
D-K: Urethritis/PID, ectopic pregnancy, neonatal pneumonia, neonatal conjuncitivis
L1-3: Lymphogranulomas venereum: small, painless ulcers on genitals that can lead to painful inguinal ulcerating lymph nodes
Mycoplasma pneumoniae Unique Feature, Presentation, and Treatment
No cell wall (not seen on gram stain)
Classic cause of atypical pneumonia (walking pneumonia). High titer of cold agglutinins (IgM). Frequently seen in military recruits and prisons
Treatment: doxycycline, fluoroquinolone, or macrolide
