What is reactive arthritis
Joint inflammation triggered by an extra-articular bacterial infection
Organisms reach joints by infected monocytes –> antigens in joint space –> arthritis
Causes of reactive arthritis
Chlamydia Salmonella Yersinia Shigella Campylobacter Clostridium
Diagnosis of reactive arthritis
Joint aspiration
Chlamydia on first pass urine
Stool cultures
Natural history of reactive arthritis
70% resolve in 6-12/12
25% have chronic relapsing course
5% destructive arthritis
Management of reactive arthritis
Rest NSAIDs Local CCSs MTX Sulfasalazine Treat sexual partners Antibiotics for post-chlamydial reactive arthritis
Disseminated gonococcal infections
Neisseria Gonorrhoea
2 different syndromes:
- Polyarthralgia, dermatitis, tenosynovitis
- Septic arthritis
F>M 3:1
Recent menstruation
10% are deficient in terminal complement components
Treat with IV ceftriaxone
Enteropathic arthritis
20% of IBD patients
Non deforming and self-limiting
Peripheral or axial
Treat IBD = treat arthritis
