Cluster headache symtpoms?
A
- occurs in clusters
- intense pain in one eye

Sinus headache symptoms?
A
- occurs when nose becomes blocked
- pain between eyes/top of the nose

Tension headache symtpoms?
A
- constant ache that feels like a tight-band around the head

Migraine symptoms?
A
- unilateral throbbing pain
- n+v
- photosensitivity/sound sensitivity
- migraine with or without aura e.g. flashing lights
- has acute triggers

Referral criteria?
A
- unresponsive to analgesics in 1 day
- taking COC with migraine-type headache
- recurring headache (medication overuse?)
- neck stiffness
- children under 12
- severe headache in >4hours
- recent trauma/injury
- increasing severity and frequency
- severe deep pain-worse when lying down (intracranial pressure from tumour?)

Sumatriptan referral criteria?
A
- 1st migraine atack in 50+yrs
- > /= 4 attacks in a month
- > 24 hour headache
- migraine symptoms have changed
- avoid in liver or renal impairment
- cardiovascular disease (hypertension, peripheral vascular disease, MI, IHD or >/= 3 risk factors for CVD)
- epilepsy

A
ANALGESICS
- paracetamol/ibuprofen/aspirin
- codeine/dihydrocodeine
ANTIEMETICS FOR MIGRAINES
- prochlorperazine (buccastem M)
- buclizine (migraleve pink)

OTC for migraine?
A
SUMATRIPTAN (18-65yrs) POM to P (5HT1 receptor agonist)
- OTC licensing: migraine with or without aura
1. previously tried simple analgesics
2. established pattern >/= 5 migrain attacks in 1 year
3. previously diagnosed by doctor or pharmacist
How to take:
1. One 50mg tablet ASAP after migraine onset
2. Do not take a second dose for the same attack
3, A second dose can be taken after 2 hours if migraine recurs (only if it had benefit after first dose)
MAX 2 IN 24HRS

Advice?
A
- daily use of analgesics can cause chronic daily headache (medication-overuse headache)
- codeine/dihydrocodeine causes addiction (no more than 3 days use)
