Calcuim and Vitamin D
What are the best sources of calcium and vitamin D?
Dietary
What do you need to look for in calcium preparations?
The amount of elemental or absorbable calcium. Calcium Carbonate has 40% absorbable calcium.
What are the side effects of calcium?
Constipation and bloating are most common.
What are some drug interactions with calcium?
Binds to antibiotic tetraclcycline decreasing/preventing absorption resulting in treatment failure
Certain foods like bran and spinach prevent the uptake of calcium
What does vitamin D do?
Facilitates the absorption of calcium from GI tract and helps control calcium homeostasis
What is the mechanism of action of the Biophosphates?
bind to hydroxyapatite and inhibit osteoclast-mediated bone resorption, without directly affecting bone formation
(fosamax and Actonel)
What are the side effects of Fosamax?
GI discomfort, musculoskeletal pain, nausea, dyspepsia
What are the adverse effects of Fosamax?
Esophageal lesions, esophagitis or esophageal ulcers. Pt must be able to stand or sit upright for 30 minutes minimum to be able to take the drug
What are the contraindications for Fosamax and actonel?
Esophageal abnormalities, inability to stand or sit upright for 30 minutes
How should Fosamax be taken?
With a FULL glass of PLAIN water
What are the side effects of Actonel?
Potential side effects are similar to Fosamax (GI problems, nausea, dyspepsia, MS pain) but at a lower incidence
What is Didronel? What is approved for the treatment of?
A diphosphonate. treatment of Paget’s disease
What are the side effects of didronel?
Mild GI problems, no risk of esophageal lesions
What are the contraindications of didronel?
Overt osteomalacia, hypocalcemia.
